Addison Healthcare Center

First of all, PT/OT here is very good. I would give them 5 stars all the way around!!! This department is their greatest asset. Nursing is fine. Most were very knowledgable and kind. I never had any problems with anybody. There is a rather large turn-around, which some people see as a red flag. Social services (Mandi) is another story. In my experience, she has NO CLUE how to work with a family as a whole. All I ever wanted was for my husband to be safe and well taken care of. My family and I had some things in motion to ensure that would be the case. That was before she decided to poke her nose in our private affairs and fill his head with CRAP. I constantly felt like I was at war with her. In my situation, the end result is that she has driven a huge wedge between my husband

So, this place gave my person the wrong medication. They had a roommate with her in for dementia and gave that person’s medication to her!!!!! I’m listed as the emergency contact for a patient being transported back to the hospital from this facility and NOBODY CONTACTED ME to let me know. Instead, they reached out to someone who is not on the emergency contact list. Their excuse, “This just happened, we were going to call you”. Okay, so why did you call someone else instead of THE ACTUAL EMERGENCY CONTACT? I was never impressed with this facility. They lock people up like animals. The entire facility is disgusting, disorganized and constantly smells like urine and feces. Procedures aren’t properly followed. The food they serve the patients is DISGUSTING! While no one place is perfect, this has been the perfect place for my father in-law who suffered a major stroke. The staff has been very caring and attentive with him and also really makes it a point to relate to him on a personal level. We truly are thankful for them all. Thank you so much to you all!

I was a resident at Addison from September 2023 until May 2025. During my stay, I have met some really wonderful people & have made close friendships with many of the residents & staff. Activities Director, Robin & activities assistants, Cloey & Nikki go above & beyond, working tirelessly for the welfare & happiness of their residents. Nursing staff- Laura, Jen, Michelle, Erin, Mike, Rakesha, Malisha, Trish , Eric & Mercy are all caring & compassionate. Nurse aides- Nick & Dominique, Wendy & Abby (just to name a few) are simply the best. Physical Therapy- Becky, Tina & Elise are all truly amazing gals. If you're in need of short term rehabilitation or long term care, the staff at Addison will make you feel at home.

All Staff (P.T., CNA'S, Nurses, Housekeeping, Kitchen, Maintenence, Front Office) were all very good. The one exception that I couldn't warm up to was the N.P.. Perhaps it was me but I can call them like I see them. An extra TY to Elise, Tina & Becky in P.T.. Place doesn't smell like many Nursing Homes. It smells clean.
