JAG Healthcare Burbank

This place is clean. People are friendly. Plan of Care meetings are held. Doctors and staff keep family up to date with any health care issues. Our mother likes this home allot better then the one we had her in before we came to Burbank Parke Care Center.

My friend Bonnie Alsatian was a patient at JAG and she was very happy with all the care and how attentive And kind everyone was. The follow up from the staff was excellent in all departments! Thank you so much for taking such great care of her!

My brother was a resident at JAG in Burbank, Ohio for over two years. During that time he received EXCELLECT care by a team of professional, caring people. When he first arrived at JAG his health was very poor but they were able to restore him to an active lifestyle. The attitude of the entire staff is very upbeat and it is easy to see that they really care about their residents. To them it is more than just a job.... it is dedication! Thanks to them my brother was able to enjoy his best quality of life. I never had to worry about anything regarding his care because i knew he was in the best place possible. From Nov 2024- Jan 2025, my mom received the most comprehensive, patient centered & evidence based care from THE BEST therapy team in the area. They coordinated her plan so that each therapist's area of specialty supported her progress in the other's, & changes were consistently implemented by direct care staff safely. Her nursing care was responsive & compassionate. They ensured any follow up needed with the facility physician was addressed quickly & communicated openly with our family to ensure we understood the information they had for us. The coordination of the practical issues of admission, progress of care, & discharge planning was very well managed. Mom has been at jag twice for rehab. The staff has always been friendly, caring, compassionate not just to her but also to our family and friends. The administration goes above and beyond with any questions and concerns we had. My aunt was also at jag until she passed. Will continue to recommend this facility.
